Your Specific Responsibilities:

  • Assist with completing strategies for the Healthy Communities of Florida workplan, including but not limited to:
    • Chronic Disease prevention
    • Go NAPSACC and FL HEROs
  • Assist with Healthy Communities of Florida reporting
  • Manage the Swim Lesson Voucher Program, including determining applicant eligibility and awarding vouchers to eligible applicants
  • Serve as Public Health Planning Division’s representative to the FDOH-Polk Workforce Development Team
    • Attend Workforce Development Team Meetings
    • Serve as an active participant in Workforce Development Team Subcommittees to assist with the completion of Workforce Development Plan objectives
  • Assist the Public Health Planning Team with Community Health Assessment and Community Health Improvement activities
  • Assist the Public Health Planning Team with Quality Improvement activities
  • Serve as health educator to assist with health fairs, community events, and presentations (which can be during OR after business hours and on weekend) on topics including but not limited to:
    • Injury Prevention (helmet fittings, etc.)
    • Chronic Disease Prevention
  • Serve as DOH-Polk representative at relevant community meetings
  • Provide administrative support to the Public Health Planning Division, including but not limited to:
    • Taking meeting minutes/conducting transcriptions
    • Compiling and submitting reports
    • Preparing and analyzing data
    • Other duties as assigned by supervisor

 

Qualifications:

  • High School Diploma
  • Valid Florida Drivers License
