Position Overview
We are currently seeking a qualified Licensed Master Social Worker (LMSW) for the role of Nursing Home Surveyor/Complaint Investigator in New York, NY . This essential role focuses on ensuring quality of care and regulatory compliance in long-term care facilities through surveys, complaint investigations, and surveillance activities.
Key Responsibilities
Conduct surveys and complaint investigations in long-term care facilities, including cases involving abuse or neglect
Evaluate nursing homes for compliance with Federal and State regulations
Draft Statements of Deficiencies (SOD) in a timely manner using the Principles of Documentation
Assess the effectiveness of Plans of Correction and ensure timely follow-up
Enter findings into state and federal compliance databases
Participate in Immediate Jeopardy monitoring and state interventions when needed
Attend relevant meetings and testify in administrative hearings as necessary
Follow all Department of Health (DOH) schedules, policies, and procedural guidelines
Travel to assigned facilities according to work schedule

Qualifications
Education & Licensure:
Active New York State Licensed Master Social Worker (LMSW) license required
Experience:
Minimum 2 years of clinical experience in a medical facility (nursing home experience preferred)
2 years of experience conducting nursing home surveillance or compliance activities (preferred)
SMQT Certification preferred (must be willing to obtain within 12 months of employment)
Skills:
Excellent investigative, documentation, and communication skills
Strong understanding of long-term care regulations and patient advocacy
Proficient in completing documentation, data entry, and report writing
Willingness to travel to on-site facilities as required
